JT Hunter, CDP is a Certified Dementia Practitioner, Care Partner Coach, and President and CEO of REMT Care Partner Coaching and REMT Media. JT began his journey as a care partner, alongside his mother, for a family member with dementia when he was only in his mid – 20’s. In that role, he witnessed the stress and challenges faced by the primary caregiver and often felt frustrated that he was unable to help more.
As fate would have it, shortly after entering the world of caregiving in his personal life, he began what would become a 10-year successful career with a nationwide nonprofit, focused on assisting caregivers and those living with Alzheimer’s or a related dementia. Leveraging his personal experience with his professional experience and training, JT developed a keen insight into the challenges facing caregivers, most specifically those who are caring for a loved one with dementia.
Now working as a Care Partner Coach, JT provides one on one caregiver coaching, care planning, and education sessions as well as family and professional consultations. Sometimes caregivers need a better understanding of the disease or condition their family member must endure. Sometimes all a caregiver needs is to know he or she is not alone. Sometimes families need a neutral third party to help them when they disagree on a plan for their loved one. JT can be your partner. He will listen. He will explain. He will understand. He will assist in outlining goals and formulating a plan that is first, in the best interests of the person needing care while considering the viewpoint of the family members as well as the realities of the disease or condition the family is facing.
JT served as the 2019 President and currently serves as the Secretary/Treasurer of the West Virginia Geriatric Society and has been a dedicated board member since 2017. JT is a 2019 Governors Service Award Winner, for his work with Dementia Friendly Communities. Over the last decade he has worked extensively with government agencies, long term care providers, advocacy groups, and community partners, to help improve the care for, and help protect the rights of all people living with dementia in West Virginia.
For more than 15 years JT has provided mediation, education, and support for both family and professional caregivers. He has been a facilitator for caregiver support groups, spoken to numerous healthcare and long term care providers nationwide, and participated in a wide variety of work groups focused on making life a little better for those with a dementia and those who care for them. JT is a highly sought-after public speaker, a nationally recognized dementia practitioner, and relentlessly works to educate, enlighten, and empower all caregivers, especially those caring for someone with Alzheimer’s or related dementia.

During these one hour coaching sessions, your coach will provide the guidance, education, and support you need, listening to your concerns, and customizing your care partner action plan to fit your specific needs. Making care plans and arrangements for the person you care for can be difficult. Sometimes different family members and friends on the care team, can have different views and opinions on what is best. We can help. Our job as mediators is to provide the expertise and knowledge of the disease, allow all voices to be heard, but ensuring the focus stays on the ultimate goal of providing the best care plan for the person you care for.

JT is recognized as a leader in his field and has extensive experience teaching classes for colleges, medical schools, long term care facilities, physician groups, as well as businesses that want to provide education, help, and support for their employees who are caregivers for someone with Alzheimer's or a related dementia.
